Who is Eligible
- Individuals with a medical need and a physician’s referral.
- Individuals requiring at least one of the following:
- Skilled nursing care
- Physical therapy
- Occupational therapy
- Speech therapy
- Homebound individuals unable to leave their home unassisted.
